Initial Therapy Paperwork-

bulletPatient Initial Treatment Packet- DTA Kennesaw Office: to be completed prior to evaluation or treatment- can be completed on-line and emailed or printed and delivered to office

AAC Paperwork

What do I need to bring to my first appointment?

  1. Completed Patient Initial Treatment Packet or Initial AAC Packet (for device evaluations)
  2. Current prescription (dated within 6-12 months) from your Primary Care Physician for the service you are obtaining
  3. Copies of your Medicaid/Insurance cards (front and back)
  4. Copies of pertinent educational/psychological/therapeutic evaluations or plans of treatment (IEPs etc).
